REQUIRED FORMS :

ALL of the following forms are required for camp. Please ensure that you fill out each form or your camper may not be properly registered. 

• M-Fuge Waiver & Release (Please complete the required fields. Laptop or Chromebook recommended. You will need the following information):
  • Registration ID : 82640
• 2024 Parkway Medical Release
• CHAPERONES ONLY:
  • Parkway Background Check Form (Due by May 19th) - OR-
  • Background Check Renewal (For returning Adult Leaders)
  • M-Fuge Child Protection Training Link (Due by May 19th). You will need to create a Lifeway account if you do not already have one. Please email a copy of your completion certificate to Karen Collie, or bring a printed certificate to the May 19th Leaders' meeting.)

IMPORTANT MEETINGS:

• Sunday, June 2 at 5:00pm  - This meeting is mandatory for campers and at least one parent.
• Sunday, June 23rd - M-Fuge Prayer & Packing Party

 FUGE PACK & FUGE STORE CARDS (optional) - Again, this year, you will be able to pre-order your student a 2024 Fuge Pack and/or Fuge Store gift cards. Order Fuge Packs and Fuge Store Gift Cards by May 26th. These items will be given to campers at camp! 

• The Fuge Pack ($35) includes a shirt, playing cards, sticker, sunglasses and a fabric bracelet.
• Fuge Store Gift Cards come in increments of $10, $25, or $50 and are be able to be used at the Fuge Store that week. *These cards will not work at other on-campus stores*

  The 2024 CAMPER CARE PACKS:
  Each pack includes an OMC Shirt, Backpack, Water Bottle, Eyeblack, Sunglasses, Luggage Tag.
  Care Packs cost $35 (a $40 value).

  The CAMP STORE GIFT CARDS:
  Store cards can be used on any items in the CentriKid Camp Store. Campers can purchase items like t-shirts, bracelets, water bottles, and more! Prices for store items will range from $1-$30.
  Sold in increments of $5, $25, or $50. (Tip: You can buy multiple of a single increment, e.g. 2x$5 cards totaling $10.)
  These are only good for the CentriKid Store and will not be not valid at other on-campus snack shops or stores. Any unused balance is non-refundable.
